LUXURY BROW CONSENT FORM

Have you ever had your brows or lashes tinted before?
Are you pregnant or breastfeeding?

If yes, please note you will be unable to have tint, lamination, or lash lift.

Are you currently using any alpha-hydroxy acids (AHAs), or glycolic products?
Are you currently using any alpha-hydroxy acids (AHAs), or glycolic products?
Are you currently using any other skin thinning products?
Are you currently using tanning beds or are you exposed to the sun often?
Do you have any moles, warts or skin sensitivities in the area to be waxed?

If you answered yes to any of the above questions, you will be unable to have any waxing treatments performed.

Do you have lupus, diabetes, do you have any auto-immune diseases?

Declerations:

I have carried out a patch test at least 48 hours prior to my appointment, and have had no reaction to the patch test
I am satisfied with the explanation of the treatment
I agree to contact my stylist immediately in the event of an adverse reaction
